Meals on Wheels
Meals on Wheels sponsored by The Prince Edward County Community Care for Seniors is available throughout the County. If you know of someone who might benefit from this service please call Community Care at 476-7493 for more information.

   
Frozen Meals on Wheels
Nutritious frozen meals are available.  Eligible clients can order any number or combination of entre, soup and dessert from an extensive menu of frozen food that is specially prepared, low sodium and includes a range of special diets. Clients place orders twice a month.
 
   

Seniors' Dinners
The Seniors' Dinners are a great oppprtunity to get together with your friends to socialize while enjoying a hot, nutritious, noontime meal. The food is prepared by Jennifer Ackerman of Celebrations Catering and served by wonderful volunteers. Meals are offered in central locations throughout Prince Edward County and are open to seniors. Why not come out and give it a try? You might meet some old friends and even make some new ones. If you require a ride you can participate in the Community Care Transportation Program. Just call the office 2-3 days in advance and we'll arrange a volunteer driver.

This program operates 12 months of the year in Picton and from October to May in the other locations.

A staff member from Community Care attends all of the dinners. They can answer your questions about what services are available through our agency, or elsewhere in our community. If you are feeling uninformed, ask the staff person at a dinner or call our office. We can answer your questions, research the answer, or direct you to someone who can help you. Home Maintenance Service
A Home Maintenance Service is available to eligible seniors by calling Prince Edward Community Care. Inside and outside work can be done. Registration must be completed in advance. Register in person at the Community Care office, 206 Main Street, The Armoury, Picton, or housebound people can call and arrange a home visit. Cost for the service is to be negotiated between the client and the worker. Call 476-7493 for more information.

   

Transportation
Community Care operates a transportation service that will assist eligible clients to go to medical appointments and shopping. Rides to destinations such as Belleville, Kingston, Trenton and Toronto for medical appointments can be arranged. To schedule a ride you must call the Community Care office at 476-7493 at least 2-3 days prior to the time of your appointment. If it is the first time that you have used our transportation program there will be an intake procedure to complete that will determine eligibility for the service. You will need to tell the office staff where you live, your appointment time and place, the number of stops you need to make, and any other necessary information.

When a volunteer driver has been scheduled for your ride you will receive a call telling you who it is and when they will pick you up. The volunteer will pick you up, take you to your appointment, wait for you and then take you home. Community Care serves eligible people who live anywhere in the entire County of Prince Edward. Friendly Visiting
Our workers will help the person become involved in programs (e.g., volunteering, Seniors' Dinners). Volunteer visitors will be arranged for shut-ins. Library books can be delivered.

   
Home Help/Homemaking
Homemakers will provide a set of willing hands to assist with routine household activities such as light housekeeping, shopping and meal preparation. Equipment and cleaning supplies to be provided by the client. Fees are negotiable. Call for information.
 
   
Seniors’ Technology Centre
The Technology Centre offers basic computer courses for seniors; small classes; with a volunteer on site to answer questions. Topics are Computer Basics; Word Processing; Internet; Email; Buying a Computer. Courses are 2 hours/day for 2 days, a total of 4 hours. Call the office for details.
 
   
Weekly Newspaper Column
The column "Information for Seniors" is in the Friday edition of the Gazette Regional and holds a wealth of information for seniors living in The County.

If you have problems, you just want to talk, or you don't know where to turn for help, call or come into the Community Care office. Experienced staff can help you- The Armoury, 206 Main St., Picton, 476-7493. Home visits can be arranged.

Assistance is available with Federal or Provincial Programs such as Old Age Security, Spouse's Allowance, Canada Pension Plan, GAINS, Supplement, Income Tax, etc. Call ahead for an appointment.

 

   
55-Alive/Mature Driving Classroom Refresher Course
Community Care offers the 55-Alive Mature Driving Classroom Refresher Course which was developed by the Canada Safety Council. This is a driver retraining program designed specifically to help older drivers improve their driving knowledge and sharpen driving ability to compensate for normal age-related changes in physical condition. There is a concentration on the physical changes of older people as they relate to vision, reading and medication. The course deals with changing road conditions, rules and regulations and the need to understand collision - prevention measures. It focuses on the major problems which are common to older drivers, which include failure to yield the right of way, improper turning, incorrect lane changing, passing, and entering and exiting highways. The sessions are made up of lectures, slide presentations, group discussions and instructor guidance. There is no actual ‘on the road' driving. Picton Foot Care
Picton Foot Care Clinic, sponsored by the Prince Edward Community Care for Seniors Association, will be held regularly at the Community Care Office located at The Armoury, 206 Main Street, Picton. Nurses with special footcare training provide the care. Call 476-7493 to arrange an appointment.

Wellington Foot Care
The Wellington Foot care Clinic, sponsored by The Prince Edward Community Care for Seniors Association, are held regularly at the Wellington United Church Hall. Nurses with special footcare training provide the care. Call 476-7493 to arrange an appointment.

   
Walking Program
Prince Edward Community Care operates a Walking Program in Picton at the high school (PECI) and in Wellington at C.M.L. Snider elementary school. Both programs operate from October to mid-May each weekday. You must register in advance by calling the Community Care office at 476-7493 or visiting us at The Armoury, 206 Main Street, Picton.


   
Telephone Reassurance Program
Community Care offers a telephone reassurance program, where a daily, 5 minute, friendly telephone call is a made to check on the well being of a senior. A volunteer will call the client every day at a pre-arranged time to check and make sure they are okay. For more information, call Prince Edward Community Care at 476-7493.
 
   
Rural Route Reassurance Program
The Rural Route Reassurance Program is a service offered by Prince Edward Community Care and supported by the Canadian Union of Postal Workers and Canada Post. Seniors who live on a rural route and wish to participate must contact Community Care to request that their mail carrier keep a friendly eye open for any signs that they might need help. Your home must be visible from your mailbox to be eligible to participate in the program. For more information or to apply for the service, call Community Care at 476-7493.

   
Seminars
Seminars are occasionally held in the office on issues such as wills, power of attorney, medication awareness, rent review, etc. Call for more information.

Family Support
Our workers will assist caregivers by listening, providing our services as a form of respite, recruiting volunteer helpers, and explaining the service delivery system. Call for an appointment.

MISSION STATEMENT:
To assist older people to live in a home environment in
reasonable independence.

Eligibility Criteria:

  • People aged 60 or over with dependencies related to: the aging process; a previous illness or injury; a sustained or periodic impairment; or a physical disability.
  • People who are age 18-59 may be eligible for services if:
    • 2.1 they have a physical disability or chronic illness;
      OR
    • 2.2 they are clients of Hospice Prince Edward or Alzheimer Society of Prince Edward County (per Memorandum of Understanding);
      AND
    • 2.3 family/friends/caregivers are not available or able to provide services;
      AND
    • 2.4 there is a demonstrated need for services from Prince Edward Community Care.
  • Services that are provided are intended to help the client to maintain their independence in the community.
  • The principal full-time residence of the client should be located in Prince Edward County.
  • Other individuals may be accepted with the recommendation of a Co-ordinator and at the discretion of the agency Executive Director.
  • Eligibility for services is based upon assessed need.

CLIENT BILL OF RIGHTS

Community Care shall insure that the following rights of persons receiving community services from Community Care are fully respected and promoted:

1. A person receiving a community service has the right to be dealt with by Community Care in a courteous and respectful manner and to be free from mental, physical and financial abuse by Community Care.

2. A person receiving a community service has the right to be dealt with by Community Care in a manner that respects the person's dignity and privacy and that promotes the person's autonomy.

3. A person receiving a community service has the right to be dealt with by Community Care in a manner that recognizes the person's individuality and that is sensitive to and responds to the person's needs and preferences, including preferences based on ethnic, spiritual, linguistic, familial and cultural factors.

4. A person receiving a community service has the right to information about the services provided to him or her and to be told who will be providing the community services.

5. A person applying for a community service has the right to participate in Community Care's assessment of his or her requirements and a person who is determined under the Long-Term Care Act to be eligible for a community service has the right to participate in Community Care's development of the person's plan of service, Community Care's review of the person's requirements and Community Care's evaluation and revision of the person's plan of service.

6. A person has the right to give or refuse consent to the provision of any community service.

7. A person receiving a community service has the right to raise concerns or recommend changes in connection with the community service provided to him or her and in connection with policies and decisions that affect his or her interests, to Community Care, government officials or any other person, without fear of interference, coercion, discrimination or reprisal.

8. A person receiving a community service has the right to be to be informed of the laws, rules and policies affecting the operation of Community Care and to be informed in writing of the procedures for initiating complaints about Community Care.

9. A person receiving a community service has the right to have his or her records kept confidential in accordance with the law.
