home first (HF)

Provides discharge and in-home supports to frail elderly patients, automatically referred to 4CSS as a component of the home first approach (Quinte Health Care, Belleville, Trenton and Picton sites; Brockville General Hospital; and Kingston General Hospital) Referral to 4CSS ensure clients receive a timely and informative offer of linkage to the CSS agency in the area of their home; all applicable fees for CSS services are waived for a 30-day period within 90 days of leaving hospital, depending on client preferences. The clients of the home first approach have been identified in the hospital and are referred by the Community Care Access Centre's in-hospital Case Managers as individuals who may require CSS supports to continue living at home or while making a decision about a move to Long Term Care.

home first Story:

The HF client was very relieved by the call from 4CSS and the offer of referral for service. My spouse is "on a treadmill" trying to provide care. The spouse worked full time and among other things would come home at lunch break to make lunch and tend to things. Along with meals, the client expressed interest in the full spectrum of CSS services. When the CSS agency made its home visit the agency co-ordinator reported that the client's concern for the spouse was front and centre and needed to be addressed as part of ensuring the well-being of the client. The following CSS services were delivered to this family over a 30-day period: MOW-hot, MOW-frozen, housekeeping, home (yard) maintenance, hair care, and respite.
